Job Description
DESCRIPTION OF POSITION:
 
The Hospice Aide is responsible for providing direct patient care under the direction of the Patient Care Supervisor or designee and the supervision of a Registered Nurse.  Assures consistent, appropriate, and cost- effective care by actively assisting in managing the patient/family’s needs, company policy compliance, and HSI’s financial objectives.
 
I.Position Requirements
  1. Education
    1. High school graduate or GED.
    2. Successful completion of Certified Nursing Assistant (CNA) course and current Illinois Certification.
  2. Experience
    1. One (1) year experience in hospice/ home health preferred.
    2. Two (2) years of CNA experience in hospital, long-term care, or home-health setting required.
  1. Other Qualifications 
    1. Current Illinois Certification.
    2. Excellent organizational and interpersonal relationship skills.
    3. Reliable means of transportation.
  2. Special Considerations
This position requires the ability to work with Adults and Geriatrics in a safe and productive manner and may infrequently work with an Infant, Child, or Adolescent.
 
  1. Technical Competencies:
    1. Adheres to policies and procedures in compliance with all state and federal regulations.
    2. Recognition of situational needs.
    3. Identify areas of risk.
    4. Organizational skills.
    5. Provide evidence of basic clinical skills including but not limited to basic vital signs, bathing, transferring, lifting, and turning patients.
    6. Accurate and complete clinical documentation.
    7. Accurate timesheet documentation.
    8. Work efficiently with minimal supervision; self-starter.
    9. Works in tandem with an Interdisciplinary Team.
    10. Collaborates effectively with internal and external customers.
    11. Willing to expand knowledge base and clinical skills.
    12. Advise PCS/Case Manager of any conflict existing with any patient/family/customer.
    13. Accurate timesheet documentation.
    14. Other duties as assigned.
