Job Description
The Social Worker, as part of the Interdisciplinary Team, is responsible for assessing, planning, and
implementing the delivery of medical social work services to hospice patients and/or families. Proactively promotes clinical competencies to assure positive patient outcomes by establishing and
participating in the use of the clinical process to meet the realistic needs of the patient and/or family, as defined by the physician’s orders and the Plan of Care. Assures consistent, appropriate, and cost-effective care by actively managing the patient’s and/or family’s needs, payor’s expectation, company policy compliance, and Hospice of Southern Illinois’ financial objectives.
a. Education
1. Masters in Social Work (MSW) or Bachelor’s in Social Work (BSW) from a
school accredited by the Council of Social Work required.
b. Qualifications
1. One (1) year experience in hospice preferred.
2. Two (2) years of experience in hospital, long-term care, or home-health setting
c. Other Qualifications
1. To be licensed in the State of Illinois as a Licensed Social Worker (LSW) or as
a Licensed Clinical Social Worker (LCSW).
2. Excellent clinical assessment, organizational, and interpersonal relationship
3. A working knowledge of Medicare/Medicaid, hospice regulations, and third party
payor expectations is preferred.
4. Reliable means of transportation.
1. Assisting in overall coordination of assigned patient-care services. Specific responsibilities include providing direct patient care activities that include all aspects of the clinical process. Adheres to all Hospice of Southern Illinois Policies, SOP’s, and guidelines.
2. Participates in Quality Assessment/Performance Improvement Program established by Hospice of Southern Illinois.
3. Documents accurate and thorough initial comprehensive psychological assessments in the electronic chart no later than 5 days after admission to hospice. Documents an updated comprehensive psychological assessment every 15 days. Completes condolence calls within 7 days of patient death.
4. Develops and updates the Plan of Care to reflect patient’s and/or family’s needs.
5. Enhances the strengths of the family unit by assuming the active role of advocate for the patient’s family.
6. Assisting supervisor with patient, office tasks and special projects as assigned
(i.e. Support calls and Making Connections).
7. Attends staff meetings, IDT, and other meetings as assigned and appropriate.
8. Sharing responsibility for 24 hour on-call needs of patients.
9. Maintaining strict confidentiality of patient and/or family information. To adhere to Hospice of Southern Illinois’ Code of Conduct. The Social Worker is expected to follow the Code of Ethics developed by the National Association of Social Workers (NASW) and the Social Work regulations established by the Illinois Department of Professional Regulations.
10. Educates patient and/or family’s spiritual and cultural beliefs.
11. Supports the patient’s and/or family’s spiritual and cultural beliefs.
12. Assumes responsibility for own personal and professional development.
13. Exhibits Hospice of Southern Illinois’ philosophy in all job related roles.
14. Maintains compliance with Infection Control policies and procedures.
15. Assures holistic family centered care across treatment settings.
16. Reports any abuse or neglect to appropriate authorities.
17. Maintains the dignity of the dying patient.
18. Provides short-term crisis intervention, individual or family counseling, and makes appropriate community referrals when indicated to assure interventions to address specific symptoms.
19. Works effectively with all members of the Interdisciplinary team.
20. Masters level LSW or LCSW provides clinical oversight and supervision to Bachelor level LSW.
21. Accurate timesheet documentation.
22. Other duties as assigned.
